The Toowoomba Hospital Foundation is the statutory authority through which donations/bequests are facilitated to the Toowoomba Health Service District incorporating the Toowoomba Hospital, Baillie Henderson Hospital, Mt Lofty Heights Nursing Home, Oakey Hospital and Gatton Hospital. To date the Foundation has facilitated numerous donations of funds for the purchase of medical equipment, staff development etc for departments/units at the hospitals.

The following bequest information, which, when included in wills, would allow for bequests to be discharged in the appropriate manner through the Toowoomba Hospital Foundation.

TOOWOOMBA HOSPITAL FOUNDATION SUGGESTED FORM OF BEQUESTS

1. GENERAL

I give and bequeath to the TOOWOOMBA HOSPITAL FOUNDATION the sum of $........... for the general purposes of the Foundation and I declare that the receipt of the Chairman for the time being shall constitute sufficient discharge to my executors and trustees AND I FURTHER DECLARE that in making such provisions I have a general charitable intention.

2. SHARES/BONDS/DEBENTURES ETC

I give and bequeath to the TOOWOOMBA HOSPITAL FOUNDATION my shares/or bonds/or debentures in ................. Limited, for the general purposes of the Foundation and I declare that the receipt of the Chairman for the time being shall constitute sufficient discharge to my executors and trustees AND I FURTHER DECLARE that in making such provisions I have a general charitable intention.

3. PROPERTY

I give devise and bequeath to the TOOWOOMBA HOSPITAL FOUNDATION my property at ................... for the general purposes of the Foundation and I declare that the receipt of the Chairman for the time being shall constitute sufficient discharge to my executors and trustees AND I FURTHER DECLARE that in making such provisions I have a general charitable intention.

4. RESIDUAL

I give, devise and bequeath to the TOOWOOMBA HOSPITAL FOUNDATION the whole/or the residue of my estate of whatsoever nature or kind and wheresoever situated, for the general purposes of the Foundation and I declare that the receipt of the Chairman for the time being shall constitute sufficient discharge to my executors and trustees AND I FURTHER DECLARE that in making such provisions I have a general charitable intention.
